• #298039 revert adding active session folder to workspace

  Reverted the change that synced the active session folder to workspace folders, so workspace folder management no longer depends on the active session.

🔨 REFACTORS

• #298030 introduce title bar actions menu and contribute changes action to it

  Introduced a new menu system in the Sessions window title bar to better organize session actions, improving extensibility for future additions.

⬆️ UPGRADES

• #298027 Update @vscode/codicons to version 0.0.45-11 and add 'new-session' icon

  Updated the @vscode/codicons package to version 0.0.45-11 and added a new 'new-session' icon to support session features in VS Code.
